Indications
Softening, Loosening, & Removing Excessive Ear Wax
Adult Dose
Otic
Carbamide peroxide 6.5%
Adult: 5 to 10 drops into the affected ear(s) twice daily for up to 4 days.
Child Dose
Otic
Carbamide peroxide 6.5%
12-18 years: 5 to 10 drops into the affected ear(s) twice daily for up to 4 days.
>2-12: 1 to 5 drops (according to patient's size) into the affected ear(s) twice daily for up to 4 days
Contra Indications
Hypersensitivity
Perforated tympanic membrane, discharge or pain, irritation or rash in the ear, or feel dizzy, use in eye
Precautions
Avoid contact with eye and mouth. If comes contact with eye, flush eye with lots of water.
Seek emergency medical attention if anyone has accidentally swallowed the medication. Not instructed to use if patient has an allergic reaction to carbamide peroxide.
Do not use if perforation or injury of ear drum exists or after ear surgery unless directed to do so
Do not generally use for >4 days

Adverse Effects
Common side effects may include a foaming or crackling sound in the ear after using the ear drops, temporary decrease in hearing after using the drops, mild feeling of fullness in the ear or mild itching inside the ear.
Rare case of dizziness, new or worsening ear problem may be seen.

Mechanism of Action
Mild mechanical loosening effect on cerumen (earwax) that has been softened by anhydrous glycerin
Following otic administration, carbamide peroxide complex releases hydrogen peroxide that breaks up the hardened wax. The hydrogen peroxide component, which further breaks down into water, is also a cerumenolytic that hydrates the desquamated sheets of corneocytes, which are the major constituent of cerumen plugs 4.
The glycerol and urea facilitates softening of the cerumen, either with or without syringing 5. Both hydrogen peroxide and urea mildly induce keratolysis with disintegration of the ear wax to help reduce the keratin-load in the ear debris and allow other active components to reach the skin under the debris 4,5.
