Indications
Respiratory Tract Infections, Peritonitis and Other Intra-abdominal Infections, Bacterial Septicemia, Infections of the Skin and Skin Structures, Pelvic Inflammatory Disease, Endometritis, and Other Infections of the Female Genital Tract, Urinary Tract Infections
Adult Dose
Parenteral
Susceptible infections
Adult: As sodium: 2-4 g daily in 2 divided doses, increased to 12 g daily in 2-4 divided doses for severe infections given as deep IM or IV infusion.
Renal Dose
Renal impairment:
1-2 g daily.
Hepatic Dose
Hepatic impairment:
Max for hepatic impairment or biliary obstruction: 4 g daily.
Contra Indications
Hypersensitivity to cephalosporin.
Precautions
History of penicillin allergy; renal or hepatic impairment; pregnancy, lactation; cystic fibrosis, clostridium difficile-associated diarrhea, hemorrhage.
Prolonged use may result in the overgrowth of nonsusceptible organisms.

Interactions
Disulfiram-like reaction with alcohol. Potentiates anticoagulants.
Potentially Fatal: Enhances nephrotoxicity by aminoglycosides and furosemide.
Adverse Effects
Skin rash, urticaria; eosinophilia, diarrhoea, nausea, vomiting; phloebitis; hypoprothrombinaemia; superinfection.
Potentially Fatal: Neuromuscular hypersensitivity; nephrotoxicity.
Mechanism of Action
Cefoperazone binds to one or more of the penicillin-binding proteins (PBPs) which inhibits the final transpeptidation step of peptidoglycan synthesis in bacterial cell wall, thus inhibiting biosynthesis and arresting cell wall assembly resulting in bacterial cell death.
