Indications

Herpes simplex, herpes zoster

Adult Dose

Adult Oral Herpes simplex: The usual adult dosage of Amenavir is 200 mg administered orally once daily after meal. Recurrent herpes simplex: 1200 mg single dose after meals within 6 hours after the appearance of the initial symptoms of recurrent herpes simplex. (Only one dose is required). Herpes zoster: The usual adult dosage of Amenavir is 400 mg administered orally once daily after meal.

Renal Dose

Renal Impairment Mild to Moderate (eGFR ? 30 mL/min/1.73 m²): No dose adjustment required. Severe (eGFR < 30 mL/min/1.73 m²): Limited data; exposure may increase — use with caution and monitor renal function. End-Stage Renal Disease / Dialysis: Not studied; avoid use or use only if benefits outweigh potential risk.

Hepatic Dose

Hepatic Impairment Mild to Moderate (Child–Pugh A or B): No significant pharmacokinetic changes; no adjustment usually needed. Severe (Child–Pugh C): Data unavailable; use cautiously or consider alternatives.

Contra Indications

Known hypersensitivity or allergy to amenamevir or any of its components. Severe Renal Impairment.

Precautions

Hepatic or renal impairment: Limited clinical data; use with caution and monitor function. Special monitoring is essential for patients with the following clinical conditions: Severe renal impairment Thrombocytopenia Elderly patients

Pregnancy-Lactation

Not Classi Pregnancy: Insufficient human data; use only if potential benefit justifies possible risk. Lactation: Limited data; small transfer into breast milk reported (estimated RID < 10%).

Interactions

Amenavir has notable interactions with certain drugs due to its metabolism via cytochrome P450 enzymes, particularly CYP3A4. Ketoconazole: As a strong CYP3A4 inhibitor, Ketoconazole significantly increases the plasma concentration of Amenavir. Rifampicin:This strong CYP3A4 inducer markedly decreases Amenavir levels, that could lead to reduced efficacy of Amenavir. Midazolam: As a substrate of CYP3A4, Midazolam's pharmacokinetics are affected by Amenavir, leading to decreased plasma levels when both are taken together. Warfarin: No significant interaction was observed between Amenavir and Warfarin, a medication metabolized by CYP2C9. Live viral vaccines: May reduce vaccine efficacy; concurrent use not recommended. Caution with drugs affecting hepatic metabolism or renal excretion.

Adverse Effects

Common: Nausea, diarrhea, headache, nasopharyngitis, sore throat. Uncommon/Post-marketing: Thrombocytopenia, gingival bleeding, palpitations.

Mechanism of Action

Helicase-Primase Inhibitor: Blocks the helicase-primase complex essential for viral DNA replication in HSV and VZV, inhibiting viral genome synthesis and replication.
